How it started
CSI Solutions developed the original Healthcare Communities Web Portal in 2005 to support HRSA’s Health Disparities Collaboratives. This groundbreaking, multi-year improvement effort engaged Federally Qualified Health Centers in every state with the aim of improving health and health care for vulnerable Americans. HRSA project leaders needed a central location to manage project logistics, share project resources, communicate across teams, and report Collaborative data. They turned to CSI Solutions to develop a platform that could manage large amounts of data and content in a secure and flexible web environment.
Where we are today
Based on the transformative experience of the Health Disparities Collaborative, CSI rebuilt and has continuously improved the Healthcare Communities portal, and it remains active more than twenty years later, supporting more than 30,000 registered users and 70 individual communities during this time. National initiatives that have been supported through the portal include CMS’s Transforming Clinical Practices Initiative, Partnership for Patients, QIO, and ESRD programs, and the Indian Health Service’s Improving Patient Care program. Numerous local quality initiatives have similarly used the portal as a vehicle for communication among participants, knowledge management, and shared learning.
The healthcarecommunities.org platform currently hosts both privately funded communities as well as a general learning community supported directly by CSI.
